Privacy Policy

1. Who we are

ProHealth Insurance Solutions is a trade name of Reagan Health Solutions, LLC, an independent insurance producer located at 1811 Crain St, Evanston, IL 60202.

Insurance is offered through licensed insurance producers. J. Reagan, licensed insurance producer, National Producer Number (NPN) 20450638.

We are an independent agency. We are not affiliated with, endorsed by, or operated by HealthCare.gov, the Centers for Medicare and Medicaid Services, the Health Insurance Marketplace, or any federal or state government agency.

This policy explains what we collect through www.prohealth-insurancesolutions.com, why we collect it, who we give it to, and what you can do about it.

2. Who this policy covers

This policy applies to information we collect from visitors to our website, from people who submit a form or request a quote, and from our clients. In this policy, you means any of those people, and we, us, and our mean Reagan Health Solutions, LLC doing business as ProHealth Insurance Solutions.

Once you become a client, this policy also serves as our privacy notice under the Gramm-Leach-Bliley Act and the insurance privacy regulations of the states where we are licensed, including 50 Ill. Adm. Code Part 4002.

3. What we collect

Information you give us. When you complete our assessment, contact form, or booking form, we ask for:

• your first and last name
• your email address
• your phone number
• your state and ZIP code
• your approximate household income range
• whether you are looking for an individual or family plan
• whether you are self-employed
• your age group
• information about medications you take, medical conditions, treatments or procedures, and recent surgeries or hospitalizations

We ask for income because eligibility for premium tax credits on Marketplace plans is based on household income. We ask health questions because some products we offer, including life insurance and certain supplemental products, are medically underwritten, and the answers affect which carriers and products you can actually qualify for.

Information we collect automatically. When you visit the site, we and our analytics provider collect your IP address, browser and device type, the pages you view, how you arrived at our site, and how long you stay. We use Google Analytics 4 for this. You can opt out of Google Analytics using the Google Analytics opt-out browser add-on at tools.google.com/dlpage/gaoptout.

Information from others. If you ask us to enroll you in a Marketplace plan, we may receive eligibility results and plan information from the enrollment platforms and carriers we work with.

We do not ask for your Social Security number, date of birth, or financial account numbers through this website. If those are needed to complete an application, we collect them through the carrier's or enrollment platform's secure system, not through a form on this site.

4. Why we use it

We use your information to:

• respond to you and answer your questions
• determine which plans and carriers you may be eligible for
• prepare and submit quotes and applications you ask us to submit
• service your policy, including renewals and plan changes
• contact you about your coverage and about open enrollment
• meet our recordkeeping, licensing, and regulatory obligations
• understand how our website is used so we can improve it

5. Who we share it with, and who we don't

We do not sell your personal information. We do not rent, trade, or share it with other agencies, lead buyers, lead aggregators, or marketing companies for their own marketing. You will not be contacted by another agency because you filled out a form on this site.

We do share your information in these limited ways, all of which are necessary to do the work you have asked us to do:

• Insurance carriers and their underwriters, when you ask us to get you a quote or submit an application. They handle your information under their own privacy policies.
• Enrollment platforms, such as the Marketplace enrollment system we use to submit ACA applications on your behalf.
• Service providers that run our business, such as our customer relationship management system, our phone and text provider, our email provider, and our website analytics provider. They are permitted to use your information only to provide services to us.
• When the law requires it, including responses to subpoenas, regulators, and law enforcement, and to prevent fraud.

If we are ever involved in a sale or merger of the business, client information may transfer as part of that transaction.

6. Health information

We treat information about your health, medications, and medical history as sensitive.

We use it only to determine eligibility and to place coverage with a carrier. We do not disclose your health information to anyone other than the carrier or enrollment platform handling your application, or our service providers acting on our behalf, without your authorization, except where the law requires or permits it.

We are an insurance producer, not a health care provider or a health plan, so we are generally not a HIPAA covered entity. Your health information is nonetheless protected under state insurance privacy law and under this policy.

7. Calls and text messages

If you give us your phone number and check the consent box on our form, you are agreeing that we may contact you about insurance by phone, text message, and email, including with automated dialing technology and prerecorded or artificial voice messages.

Consent is not a condition of purchase. You can buy coverage through us without agreeing to automated calls or texts. Just tell us how you would rather be reached.

You can revoke consent at any time by replying STOP to any text, by telling the person you are speaking with, by emailing jay@prohealth-insurancesolutions.com, or by calling 833-406-1615. Message and data rates may apply. We honor federal and state Do Not Call registries for marketing calls; we may still contact you about a policy you already have with us.

8. Cookies

Our site uses cookies and similar technologies for basic site function and for analytics. Most browsers let you refuse or delete cookies in your settings. Refusing cookies may affect how parts of the site work.

9. How we protect your information

We limit access to your information to the people who need it to do their work, we use reputable vendors with their own security controls, and our website is served over an encrypted connection. No method of transmission or storage is completely secure, and we cannot guarantee absolute security.

10. How long we keep it

We keep client records for as long as you are a client and afterward for the period required by insurance recordkeeping and licensing rules in the states where we are licensed, generally at least five years. We keep inquiry and quote records for a shorter period unless you become a client.

11. Your choices

You can, at any time:

• Ask what we have. Request a copy of the personal information we hold about you.
• Correct it. Ask us to fix anything inaccurate.
• Ask us to delete it. We will delete what we are not required to keep by insurance recordkeeping rules.
• Stop the contact. Ask us to stop calling, texting, or emailing you.

Because we do not sell or share your personal information for cross-context behavioral advertising, there is nothing for you to opt out of on that front. Residents of states with comprehensive consumer privacy laws, including California, Colorado, Connecticut, Virginia, Texas, Oregon, and Montana, have the rights listed above, and may appeal a denial by replying to our response. We do not discriminate against anyone for exercising a privacy right.

12. Children

This site is not directed to children under 13 and we do not knowingly collect information from them. Information about children may appear in a household application that a parent or guardian submits; we treat it the same way we treat other application information.

13. Links to other sites

Our site links to carrier sites, enrollment platforms, and scheduling tools we do not control. Their privacy practices are their own. We encourage you to read them.

14. Changes to this policy

If we change this policy we will post the revised version here and update the date at the top. Material changes affecting how we handle information we already hold about you will be communicated to clients directly.
